Requirements :
Minimum Education Requirement : Graduate of an Accredited Medical School.
Minimum Experience Requirement : Supervision of staff preferred. Residency completed and one year of fellow cases in Urology preferred.
Minimum License Requirement : Current West Virginia Licensure. Current Federal Drug Enforcement Agency Certificate. Board eligible required. Board Certified preferred. Fully credentialed through medical staff according to Neurology servic
