$750.00 Sign-on bonus - $19.48 - $22.56 per hour
MDS is a non-profit organization designated by the State of NH to assist individuals of all ages with developmental disabilities to live, work and learn in their communities.
Adult Service Coordinator Case Managers are advocates serving people with developmental disabilities and traumatic brain injuries. This position is to join a growing team of 15 coordinators to work with adults with developmental disabilities. The position is based out of our Keene office. The work week is Monday - Friday, 37.5 hours per week.
Adult Services Coordinator Responsibilities:
- Advocates/coordinates services on behalf of individual and their family/guardian.
- Ensures all services are designed to promote individual's personal development, independence and quality of life in a manner that is determined by the individual and his/her family/guardian.
- Provides/assists families in accessing family supports and services, including providing information to families about the regional Family Council.
- Develops, leads and facilitates meetings to assist individuals and their family/guardian.
- Maintain monthly contacts with individuals either verbally or in-person meetings.
- Facilitates/coordinates development of a person centered Individual Service Plan (ISP) or Individual Service Agreement (ISA) according to the needs and desires of the individual served.
- Quarterly meetings with individual to determine if the ISP/SA meets the individual's needs, interests, satisfaction and terms of service agreement are being met.
- Maintains accurate and up-to-date notes in service lots of individual's services on a monthly basis.
- Monitors individual's service authorizations in NH Easy (state system) and requests for additional services.
- Provide safeguards by intervening or reporting when ISA/SA are not being appropriately honored or when a person's life and/or decisions are unsafe.
Adult Service Coordinators (ASC's) work in partnership with individuals, their families, guardians, staff, subcontractors and the community to plan, coordinate, facilitate and monitor the implementation of services and supports for individuals who are eligible for services from area agencies.
Adult Services Coordinator Requirements:
- strong advocacy skills
- ability to work with a variety of teams
- experience with meetings facilitation
- excellent writing and documentation skills
- negotiation of budgets and problem solving
- conflict management skills
- a Bachelor’s is preferred and a minimum of 2-4 years of experience in human services
- Must have experience with supporting individuals with disabilities
- Criminal, DMV and other background checks are required
