What are the responsibilities and job description for the Case Manager / Adult Service Coordinator position at Monadnock Developmental Services?
Summary
Adult Service Coordinators (ASC) are advocates serving people with developmental disabilities and traumatic brain injuries. ASC's work in partnership with individuals, their families guardians, staff, subcontractors and the community to plan, coordinate, facilitate, and monitor the implementation of services and supports for individuals who are eligible for services from area agencies.
Essential Functions
- Advocates for and coordinates services on behalf of the individual and their family and guardian (if applicable) to ensure that all services are designed to promote the individual's personal development, independence and quality of life in a manner that is determined by the individual and his / her guardian / family.
- Provides or assists families in accessing family supports and services, including providing information to families about the regional Family Council.
- Ensures that the environment or setting in which an individual receives services promotes the person's freedom of movement, ability to make informed decisions, self-determination, safety, participation in the community, employment and connection with family and friends.
- Provides support in helping individuals and their teams make decisions and provides education regarding what resources are available within the community.
- Provides assistance in developing a network of supports from community, family, and friends in order to enable individuals to live and work more naturally in their community.
- Develops, leads and facilitates meetings to assist people in their lives and maintains contact with an individual on a monthly basis either verbally or in-person and documents such meetings.
- Facilitates and coordinates development of a person-centered Individual Service Plan (ISP) or Individual Service Agreement (ISA) according to the needs and desires of the individual served. Writing must be completed within 10 days after the meeting.
- Quarterly, or more frequently if needed, meet with individuals to determine if the ISP / ISA meets the individual's needs, interests, satisfaction, and terms of service agreement are being met.
- Maintains accurate and up-to-date notes in service logs of individual's services on a monthly basis.
- Monitors individual's service authorizations in NH Easy (state system) and requests for additional services.
- Encourages self-advocacy and discourages dependency on the system.
- Provides safeguards by intervening or reporting when ISP?ISA are not being appropriately honored or when a person's life and / or decisions are unsafe.
- May assist with coverage or provide crisis intervention as part of the department on-call notifications.
- May assist with coverage or practice crisis management, develops skills to effectively facilitate a crisis when they occur.
- Assess needs for new supports, modifications and services within the region or statewide if necessary.
Education / Experience
Bachelor's degree from an accredited college or university in human services or related profession. Minimum of three years of experience providing services to individuals with developmental disabilities, must possess excellent communication skills.
