Substance Abuse Group Facilitator/Case Manager
Essential Job Duties:
- Provides professional group rehab and educational groups as part of the consumer’s treatment programming
- Provides linkage, referral, and advocates for consumers in obtaining needed resources and services.
- Serves as a member of the Treatment Team Staff to provide input on consumers regarding referrals, child custody, day care, housing, legal status, and other pertinent information and presents weekly updates/progress of each consumer as warranted.
- Arranges transportation or transports to different community appointments and outings.
- Responsible for knowing and/or coordinating available resources in the areas of substance abuse treatment, child and family services, education/employment, housing, and social services in the community.
- Provides networking services to assist in integration of consumers into the community through provision of information and referrals.
- Develops and maintains a resource file on available resources in the areas of substance abuse treatment, child and family services, education/ employment, housing, and social services in the community.

- Provides psychosocial educational groups.
- Maintains inventory of personal hygiene and/or clothing items for indigent consumers.
- Complete on-call duties for the months assigned
- Must be familiar with and follow MONARCH, ODMHSAS and CARF Policy and Procedures.
- Maintain strict confidentiality concerning all consumers and records protected under Federal Regulation.
- Documents Case Management information on each consumer to include court documents, correspondence with social services and all application for supplemental services (housing, social security, disability, food stamps, etc.) and any other material pertinent to the consumer’s case.
- Maintains files/records of each consumer as outlined by CARF, ODMHSAS, and/or MONARCH Standards and Criteria.
EDUCATION/QUALIFICATIONS:
Minimum Bachelor’s in a behavioral health related field which includes but is not limited to psychology, social work, occupational therapy, family studies, human resources/services counseling, human developmental psychology, gerontology, early childhood development, chemical dependency studies, school guidance and counseling, rehabilitative services, sociology, or criminal justice, earned from a regionally accredited college or university recognized by the United States Department of Education, a current license as a registered nurse in Oklahoma, or a current certification as an alcohol and drug counselor in Oklahoma.
CERTIFICATES, LICENSES, AND REGISTRATIONS: THIS POSITION REQUIRES THE FOLLOWING CREDENTIALS:
- Must maintain all applicable certifications and/or license ·
- CPR and First Aid ·
- Valid state driver’s license in good standing
- Required vehicle Insurance
