- Provide comprehensive clinical care for patients with multiple comorbidities including CKD primarily via telehealth with occasional in-home visits.
- Monday – Friday 8 am-5pm schedule with voluntary on-call coverage to 10 pm through Monogram Health First clinical program.

- Oversee population health management initiatives, including risk stratification, care coordination, and patient engagement strategies to improve health outcomes and reduce healthcare disparities.
- Utilize data analytics and performance metrics to track and monitor key quality and cost indicators, identify areas for improvement, and drive continuous quality improvement.
- Treat patients as the direct provider in collaboration with APPs and interdisciplinary team, providing evidence-based care including complex care through pharmacological and non-pharmacological interventions.
- Lead the development and implementation of clinical guidelines, protocols, and best practices to standardize care delivery and promote evidence-based medicine.
- Collaborate with executive leadership, clinical teams, and external stakeholders to develop and implement value-based care strategies that improve patient outcomes, reduce costs, and enhance the overall quality of care.
