Position: Regional Lead Pharmacist
The Regional Lead Pharmacist will implement and manage Monogram’s medication therapy management (MTM) program which promotes member-specific appropriate use of medications through collaboration with medical providers serving our members. The Regional Lead Pharmacist will manage pharmacy team members and work with Monogram clinical and operational departments and treating community and Monogram Health medical providers to promote and deliver appropriate drug therapy and improve member health outcomes. The Regional Lead Pharmacist will ensure integration of evidence- based medication management into Monogram’s care model and assist field clinicians in a defined geographic region execute and manage our MTM program consistent with MH procedures and operational and quality standards.

Roles And Responsibilities
Time Allocation: 35%
- Provide MTM and operational support to meet department and company goals
- Conduct comprehensive medication reviews and medical provider and member outreaches to ensure individual patient, pharmacist and pharmacy teams goals are met.
- Serve as a pharmacist resource for Monogram field clinicians and participate in external and internal interdisciplinary care team (POD) meetings and produce MTM related documentation shared with appropriate treating providers and MH care teams.
- Monitor, and manage performance standards for a defined region’s clinical pharmacist team members and ensure CMR, provider outreach and member outreach deliverables are met consistent with MH goals and client requirements.
- Lead pharmacist team members
- Ensure individual pharmacist meet performance productivity and clinical quality standards in work products such as CMRs, Provider & member outreaches
- Assist with orientation of new pharmacists in designate region
- Execute mentorship program and manage clinical skills assessments for clinical pharmacists within the region consistent with evidence-based guidelines and best practices.
- Work collaboratively with pharmacy technician team and leader to support MTM activities.
- Provide region’s care managers and MH providers operational and clinical training and support to meet the needs of patient served by the care teams (PODs)
- Support pharmacy leadership in demonstrating value of Monogram pharmacy program
- Assist with refinement of medication recommendation outcome reporting.
- Identify and deploy education and materials for pharmacy technicians as needed to support pharmacist efforts as requested by pharmacy leadership.
- Serve as point of contact for Regional Operations Director, Medical Director and President
- Support Monogram leadership and clinical departments as necessary with any tasks required to deliver excellent personalized care and perform all other duties as assigned
- Oversee and uphold the mission and values of Monogram Health in all respects
- Office-based and located in Nashville, TN or Tempe, AZ with some domestic travel
- Bachelor’s degree in pharmacy or PharmD
- Current pharmacist license
- 7 years previous experience (or equivalent) actively practicing as a pharmacist, preferably within a managed care, ambulatory care, or clinical practice environment
- Desired qualifications include:
- PGY1 residency in ambulatory care, managed care, or related clinical field
- BPS board certification in pharmacotherapy, geriatrics, ambulatory care, or related clinical field
- Board certification in MTM
- MTM certificate training from APhA, Powerpak, or similar program
