Responsible for providing patient care services. Makes planned and unplanned visits as assigned. Responsible for implementing plan of care consistent with standards of practice. May assess patients and families who request admission to Montgomery Hospice to determine appropriateness for admission to hospice.
Duties And Responsibilities
Provides patient care services, planned and unplanned.
Ensures implementation of plan of care.
Completes all documentation using electronic medical records system.
Coordinates with other disciplines to ensure appropriate services.
Minimum Qualifications
Graduation from an accredited nursing program. B.S.N preferred.
Current State of Maryland Nursing License.
Two years of recent clinical experience, preferably with critical care or previous experience in a hospice or homecare program.
Requires Strong Critical Thinking Skills And EMR Experience.
Possess excellent verbal and written communication skills, and ability to communicate effectively with patients and families under stress.
