Responsibilities
- Conduct comprehensive assessments of patients to determine their needs and develop individualized care plans.
- Collaborate with nursing staff, physicians, and other healthcare professionals to ensure optimal patient outcomes.
- Maintain accurate and up-to-date medical records in compliance with healthcare regulations.
- Utilize electronic health record systems such as Epic or Cerner for documentation and data management.
- Monitor quality metrics such as HEDIS to improve patient care standards.
- Participate in utilization management activities to ensure appropriate resource allocation.
- Provide education and support to staff regarding MDS processes and documentation requirements.
- Assist in the coordination of care for patients in specialized units such as PICU or Level II Trauma Centers.
Requirements
- Bachelor’s degree in Nursing, Healthcare Administration, or a related field preferred.
- Previous experience in a hospital or clinic setting is highly desirable.
- Familiarity with MDS processes, medical records management, and patient care protocols.
- Proficiency in electronic health record systems (Epic, Cerner) is a plus.
- Knowledge of HEDIS measures and utilization management practices.
- Strong communication skills and the ability to work collaboratively within a team environment.
- Attention to detail and strong organizational skills are essential for managing multiple tasks effectively.
