Description:
Position Summary: To train, coach and support the individuals we serve to promote the highest level of independence, and to assist these individuals to learn new skills.
Essential Responsibilities:
- Responsible for remaining on shift until adequate coverage can be provided as determined by House Manager or designated responsible person (Senior Person on shift).
- Responsible for consistent implementation of resident programming training (Service Agreement Goals & Objectives: daily living skills, i.e. bathing, dressing, meal preparation; community, access i.e. shopping, leisure activities, concerts, sports, etc.).
- Will provide transportation for residents in vehicles approved by Agency. In emergency situations, staff may be required to transport clients in their own vehicles.
- Responsible for meeting all Residential Standards and Regulations as required by the Division of Mental Health and Developmental Services and/or Moore Center Services, Inc.
- Will participate in the development of resident’s programs, i.e. Service Agreements, Behavior Programs, Strategies and Client Centered Conferences.
- Responsible for aiding in the general upkeep of residence, furnishings and equipment (yard/lawn work, mop, dusting, vacuuming, laundry, housekeeping)
- Upon successful completion of medication training program within one month and subsequent re-certification employee is responsible for supervision of residents’ self-administration and/or the distribution and administration of prescribed medications to residents as designated by the House Manager.
- Responsible for supervision of residents’ medical/evaluative appointments and accompaniment of residents to such appointments.
- Responsible for appropriate record keeping and report writing, i.e. Daily Log content, Resident Goal and Objective documentation, Significant Event Reports, Accident Reports, Behavior Programs, filing along with other reports as needed.
- Will attend the required meetings as requested.
- Responsible for reporting any pertinent client incidents/inconsistencies from routine.
- Staff working in designated residences will complete training behavior management programs prior to working with specific residents. Staff are responsible for consistently implementing and documenting interventions as indicated in programs. See addendum for physical requirements.
- Responsible for always ensuring resident hygiene and appropriate attire.

Qualifications:
Education and Experience
- High School diploma or equivalent
- A minimum of 1 year in human services or health care required
- Proficiency in Microsoft Office Suite and database systems
- Clean criminal, motor vehicle and Bureau of Elderly and Adult Services background checks
- Valid driver’s license, 100,000/300,000 liability insurance and reliable vehicle.
