Case Manager III - Remote - RN (NYS), LMSW or LCSW
Position type - Temp to Perm with high likelihood of Perm.
This position is 100% remote - Candidates need to have a NY License or be willing to obtain by reciprocity
Pay: $45-$48/hr
Summary:
Responsible for managing and coordinating healthcare services for the well-esteemed healthcare organization's members to achieve optimal clinical, financial, and quality of life outcomes. Conducts case management for members with chronic or complex conditions, proactively identifying and assessing their needs. Collaborates with members, caregivers, physicians, and other healthcare professionals to develop and implement integrated care plans. Monitors and evaluates the effectiveness of care plans, ensuring compliance with the organization's policy and all relevant regulations. Promotes service integration, conducts face-to-face or home visits as needed, and maintains department productivity and quality measures. Demonstrates strong communication and organizational skills, adheres to documentation guidelines, and participates in interdisciplinary care team meetings. Possesses knowledge of applicable regulations, coding, and case management tools. Maintains positive relationships with the provider community and internal/external customers.
Requirements:
Candidates must be able to fluently speak any of the following languages: Mandarin, Cantonese, Korean
Candidates MUST be licensed - have to be NYS with Behavioral Health experience
Licensed LMSW and LCSW is acceptable
Experience with Geriatric Population in preferred but not mandatory
