The primary purpose of the Social Worker position is to provide resident services for Health and Wellness Center residents and support to resident families and responsible parties. The Social Worker meets the social service requirements of federal and state regulatory agencies; assists with admission, transfer and discharge planning within the community and from outside referral sources; in conjunction with direct supervisor, oversees external consultants as they provide specialized assistance to residents. Serves as a clinical leader under the RN clinical coordinator assisting with oversight and management with the daily supervision of nursing staff and coordination of all nursing activities to ensure the highest quality of care.

What will I do in the role of Social Worker?

• Advocates for resident needs and promote resident rights. Works with families and responsible parties to assure resident’s personal, safety and psychosocial needs are met.
• Participates in Resident Care Assessment process and actively screens or assess the social, medical, emotional and financial needs of residents upon resident admission, readmission and when there is significant change in residents overall status.
• Completes all necessary documentation to comply with federal and state regulatory agencies, and department standards including the MDS.
• Coordinates and participates in Resident Care Plan processes and discharge planning.
• Insures that all DME needs are met for residents returning home or to another level of care.
• Assists families with coordinating outside services, if needed.
• Provides relief coverage in the absence of the Admissions Coordinator and cover admissions on-call every other weekend from Friday at 5pm to Monday at 8am.
• Coordinates and document admission 48 hour plan of care and discharge planning with Interdisciplinary team, resident and resident representatives.
• Coordinates and participates in Resident Care Plan processes and discharge planning.
• Issues and processes Medicare beneficiary information, NOMNC, and ABN as needed.

What do I need in the role of Social Worker?

• Master’s Degree in Social Work or related degree in an accredited college or university.
• Experience working as a Social Worker in an older adult health care setting, preferably in Long Term Care for 2 years.
• Understanding of best practice related to Dementia care, Behavior management, Diversity and Inclusion initiatives in the market
• Ability to accurately assess social, mental, and emotional status of older adults.
