Brewer, Maine | Full-Time | Starting at $26.00/hour
Are you passionate about making a lasting impact in the lives of children and families? Morrison Center is seeking a compassionate, dedicated, and knowledgeable Children’s Case Manager to join our dynamic, multidisciplinary team in Brewer, Maine.
In this vital role, you’ll support children from as young as 6 months to 20 years old who have intellectual and developmental disabilities. Your work will empower families, coordinate life-changing services, and help shape brighter futures—one plan at a time.
What You'll Do:- Develop individualized support plans through person-centered planning tailored to each child and family.
- Monitor and analyze service strategies to ensure they align with the unique needs and goals of the child.
- Serve as a liaison between families and community agencies, representing Morrison Center with professionalism and care.
- Conduct monthly home visits, school observations, and attend IEP meetings, team meetings, and medical appointments to provide comprehensive, wraparound support.
- Ensure compliance with regulatory requirements (including DHHS), maintain documentation, and manage service timelines (including 90-day continued stay reviews).
- Bachelor’s degree required
- Experience with intellectual and developmental disabilities (IDD) preferred
- Excellent communication, organization, and collaboration skills
- A commitment to family-centered care and advocacy
