Hours : 4pm - 11pm
JOB DESCRIPTON : The concierge is a vehicle to transport information from the patient and / or family to our interdisciplinary team for appropriate follow up and resolution. Additionally, this information obtained through concierge interaction with patients and / or family may be valuable to the interdisciplinary team in establishing a resident-centered care plan with individualized interventions to improve patient outcomes and psychosocial well-being.
- Upon admission a patient is assigned a personal concierge
- Initial admission, 24 hour, and weekly, monthly and discharge resident satisfaction surveys
- Conducts 1 : 1 resident interview
- Communicate with patient and / or family as needed throughout their recovery
- Develop, in conjunction with Administrator and other Department Managers, policies and procedures to improve processes that will improve the experiences of our patients and our staff
- Serves as a listening ear through one-to-one visits as needed
- Follows patient throughout their care experience, paying attention to detail
- Provides customer service and resident advocacy
- Create and submit monthly case studies
- Perform other duties as assigned.
