The Geriatric Physician is responsible for delivering high-quality medical care to residents at the PACE Center and other designated facilities. This role involves providing leadership on medical issues, trends, and staff training across the health system.
ESSENTIAL DUTIES AND RESPONSIBILITIES
• Provides comprehensive patient care to patients of Palm Beach PACE, while in the PACE setting, community, and other designated facilities.
• Collaborates closely with the Interdisciplinary Team (IDT) to manage and oversee the medical care of residents and patients, including but not limited to, engagement in the IDT daily meetings, involvement in care plan meetings with participants and family members, and the development of integrative care plans.
• Collaborates closely with the medical team to manage coordination of care across all settings.
• Monitors and ensures the achievement of key quality outcomes; develops and implements protocols to maintain high standards of care.
• Leads and participates in quality improvement programs within Palm Beach PACE.
• Addresses the unique and evolving needs of complex and frail elderly patients.
• Provides rotational on-call coverage in collaboration with other physicians.
• Develops and delivers training sessions and educational resources to support Advanced Practice Registered Nurses (APRNs) in staying current with the latest medical practices and advancements.
• Performs all other duties as assigned.
SUPERVISORY RESPONSIBILITIES
• This role provides supervision to the APRNs, offering guidance on complex clinical cases and ensuring adherence to best practices and regulatory standards.
QUALIFICATIONS EDUCATION and/or EXPERIENCE
To perform this job successfully, an individual must be able to perform each primary function satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
• Graduate of accredited family medicine or internal medicine residency training program and board certified/board prepared with the American Board of Medical Specialties.
• Must meet threshold criteria and qualifications for credentialing and privileges.
• Proven experience in providing comprehensive care to elderly patients.
• Strong leadership and teamwork skills.
• Commitment to continuous quality improvement and patient-centered care.
LANGUAGE SKILLS
Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the public. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.
CERTIFICATES, LICENSES, REGISTRATIONS
• Physician Licensed in the State of Florida.
• Board certification in Geriatrics or related specialty preferred.
