Reporting to the Mosaic at Home Manager, the primary focus of this role is to ensure the individuals served by contractors in home environments are compliant with state rules, regulations and Mosaic’s quality standards and requirements. - Engage, build and maintain positive relationships with individuals, home providers, family members and others stakeholders
- Assist persons supported in effectively communicating things that are important to them, such as goals and dreams, as well as important for them. Monitor the relationship between the person served and home provider or direct care worker to ensure it continues to be a successful match.
- Participate as a member of the service planning team to develop an Individual Support Plan representative of the person's supported wishes (important to them) and needs (important for them) consistent with policy and regulatory requirements.
- Oversee service delivery and contractual compliance through timely review and approval of documentation, complete visits in the home environment (Mosaic at Home).
- On call to provide assistance Mosaic at Home providers SCHEDULE : Days, Salaried, OnCall responsibilities to support contractors / individuals which can involve evenings and weekends based on need. Travel required approx 2 hours with vehicle or mileage reimbursement provided. - Strong verbal and written communication skills
- A positive, high energy and solution oriented mindset
- Excellent listening, attention to detail, problem-solving and organizational skills
- Two years of work experience preferably with people with intellectual disabilities
- A love of operations and creating seamless, efficient environments
