Position Title: Case Manager
Responsible to: Assistant Executive Director
Summation of Job Responsibilities:
- Resident Support & Advocacy: Assist the management team in delivering comprehensive services to residents, ensuring they receive the necessary resources, care, and guidance.
- Education & Employment Coordination: Work closely with the Program Coordinator to support residents in securing educational opportunities and employment, helping them achieve their personal and professional goals.
- Transportation Management: Arrange and provide transportation for residents to planned activities, educational classes, job interviews, and emergency situations.
- Professional Meetings & Appointments: Accompany residents to important professional meetings, medical appointments, and legal proceedings to provide support and advocacy.
- Financial Oversight: Manage and oversee residents' allowances, ensuring responsible financial practices and adherence to program guidelines.
- Intake & Assessment: Conduct new resident interviews, complete intake assessments, and make referrals as necessary to ensure residents receive appropriate care.
- Record Keeping & Documentation: Maintain thorough and accurate client records, complete all necessary paperwork, and communicate effectively with professional teams and external agencies.
- Goal Setting & Progress Monitoring: Collaborate with residents to establish and monitor weekly SMART goals, providing guidance and encouragement to ensure continued growth.
- Shelter Assistance: Provide hands-on assistance to residents in need, fostering a safe, welcoming, and supportive environment.
- In-House Programming: Assist in the development and implementation of educational and life skills classes, including parenting, birth preparation, nutrition, and general life skills.
- Staff Support & Coverage: Step in to cover duties of absent staff members when needed, ensuring seamless operations and resident support.
- Resident & Child Welfare: Ensure each resident and their child have appropriate, seasonal clothing in good condition, coordinating with external resources to provide additional necessities.
- Service Navigation: Guide residents through the process of applying for essential support services such as financial assistance, food stamps, and medical coverage, and accompany them to agency appointments.
- Other Duties as Assigned: Perform additional tasks and responsibilities as required by the Assistant Executive Director to support program success.
Qualifications for the Position:
1. Education: Bachelor’s degree in Psychology, Social Work, Counseling, or a related field.
2. Experience: Minimum of two years in case management preferred. Experience working with vulnerable populations, including teenagers, single parents, and homeless individuals, is highly desirable. Familiarity with newborn and infant care is preferred.

4. Background Checks: Must pass a Child Abuse/Neglect and Criminal Background Check.
5. References: Provide three positive written references.
6. Age Requirement: Must be at least 21 years old.
7. Driver's License: Must hold a valid driver’s license with an acceptable driving record and current car insurance.
Requirements for the Job:
- Hours: Full-time, 40 hours per week, with on-call responsibilities as needed.
- Fieldwork Expectation: At least 80% of working hours should be spent in direct engagement with clients, with up to 20% dedicated to documentation and administrative tasks.
- Physical Requirements: Ability to climb stairs, lift at least 50 pounds, and care for infants, toddlers, and residents without assistance.
- Dress Code: Professional and appropriate attire is required for office work, meetings, and public representation of the organization.
