MDS Assessment Coordinator (LPN / RN)

Location :  Mount Carmel Senior Living Community, Mt. Carmel, PA

Mount Carmel Senior Living Community is seeking an experienced and detail-oriented  MDS Assessment Coordinator (LPN / RN)  to join our team. This critical role ensures accurate resident assessments and compliance with state regulations while supporting individualized care plans aligned with our facility's standards.

Position Details

  • Schedule :  Full-Time
  • Location :  Skilled Nursing Facility, Mt. Carmel, PA

Key Responsibilities

  • Conduct resident assessments per schedule or as needed for condition changes and hospital returns.
  • Document and communicate condition changes accurately to appropriate personnel.
  • Collaborate with interdisciplinary teams to complete MDS, CAAs, and care plans, ensuring compliance and proper documentation.
  • Prepare and manage MDS schedules for the care team.
  • Transmit MDS data to the state and maintain accurate records, including transmittal logs.
  • Develop and regularly update care plans based on resident needs.

    • Qualifications

  • Active  LPN  or  RN  license in Pennsylvania.
  • Minimum  1 year  of experience as an MDS Assessment Coordinator in a long-term care setting.
