- Establish and maintain relationships with nursing home staff, hospital treatment teams, and community service boards to facilitate collaborative care.
- Design and implement evidence-based discharge protocols to optimize patient outcomes and reduce hospital readmissions.
- Conduct thorough needs assessments to identify barriers to discharge and develop targeted interventions to address these challenges.
- Provide ongoing support and guidance to nursing homes and other facilities serving at-risk geriatric populations.
- Maintain accurate records and documentation through electronic health records, adhering to regulatory standards.
Requirements:
- Thorough knowledge of behavioral health interventions and strategies for geriatric populations.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to work independently and as part of a team, prioritizing tasks and managing multiple projects simultaneously.
