Main Responsibilities:
- Establish strong relationships with nursing home staff to ensure seamless coordination of care.
- Collaborate with treatment teams to develop comprehensive discharge plans, ensuring smooth transitions for patients.
- Communicate effectively with various stakeholders, including consumers, family members, staff, referral sources, and the general public.
- Manage multiple priorities and projects, utilizing exceptional time management skills and creative problem-solving abilities.
Qualifications:
- Master's degree from an accredited college or university in a human services-related field or healthcare administration.
- Experience working with geriatric or mental health populations preferred.
- Licensed Mental Health Professional status a plus.
