Position Responsibilities :
- Provides screenings and assessments of new patients for both Partial Hospitalization and outpatient services, both via telephone and in person.
- Devises, completes, and documents on a regular basis on the initial treatment plan, master treatment plan, integrated diagnostic summary, and treatment plan updates.
- Facilitates group therapy sessions and 1 : 1 sessions on a daily basis as needed.
- Establishes and maintains effective interpersonal relationships with the patient, work team, and all levels of the organization.
- Follows Patient Safety goals
- Completes daily tasks as assigned, completes all clinical documentation from group or individual therapy sessions on the day of service.
- Completes Biopsychosocial assessment within 48 hours of patient admission.
- Reports incidents to supervisor and Risk Management department within required timeframe in accordance with medical center policy.
