Compensation
Mount Sinai South Nassau provides a salary range in good faith determination of potential compensation to comply with the New York State law on Salary Transparency in Job Advertisements. The Transitional Care Coordinator follows the hospitals and department’s mission to meet its objectives to ensure a patient’s safe transition from the hospital to the next level of care including their home, long term care facility or rehab. The Transitional Care Coordinator strives to facilitate the patients’ discharges by assuring authorization and acceptance for the services required in the community, such as, rehab, long term, home care, MLTCs, transportation.
Education Required
- Must complete in-house orientation program
- High school diploma required
- Associates Degree preferred
Experience And Skills
- Excellent interpersonal skills
- Speaks, reads and writes English
- Prior experience in a health care setting is preferred.
- Ability to work in an organized and professional setting
- Requires excellent oral/written communication skills
- Working knowledge of Microsoft Office (word/excel) and other appropriate computer software.
- Organizational skills
- Detailed Oriented.
- Previous customer service experience.
- Prior office or clerical experience or completion of medical billing/coding courses
- Knowledge and experience with managed care and third party insurance coverage highly recommended.
- Typing, filing, and accurate data entry skills.
License/Certifications
