Full Job Description
I. Job Summary:
Responsible for the planning, development, organization, implementation and evaluation of case management activities/individual program plans for assigned caseload.
II. Job Duties and Responsibilities:
The statements below describe the general nature and level of work being performed in this role. They are not intended to be a complete list of all duties and additional responsibilities may be delegated as required:
1. Plan, develop, implement, supervise and evaluate IPP and overall plan of care for assigned resident. Ensure validity and appropriateness of care and programs received/attended by residents.
2. Advocate for residents - work with housemothers/managers to ensure residents' needs are met and rights upheld.
3. Coordinate annual assessments for caseload, determining strengths and needs with inter-disciplinary team (IDT) and inform all appropriate departments/staff of results.
4. Complete required paperwork on a timely basis.
5. Coordinate acquisition of appropriate training material and supplies necessary for program implementation.
6. Coordinate as needed training for implementation of extablished programs.
7. Monitor progress and implementation of programs in residents' residential area and vocational programs (which includes on-site observations).
8. Collect and review data on a minimum of a mothly basis to assure accurate reporting. Intervene when necessary to assure proper documentation and implementation.
9. Document QIDP monthly/progress notes, adjusting programs as needed.
10. Coordinate monthly meetings with housemother/house manager and appropriate residential staff.
11. Assist housemother/house manager with ensuring adequate supervision.
12. Intervene during resident maladaptive behavior episodes when available.
13. Follow up, as necessary, in coordination with housemother/house manager regarding all resident concerns and needs.
14. Develop and implement behavior support programs.
15. Other duties as assigned by supervisor.
II. Qualifications:
Education:
1. QIDP Certified Preferred or BA/BS in Human Relations field
2. One year experience with IDD population (preferred)
3. Must maintain current CPR/First Aid certification and demonstrate ability to respond appropriately in an emergency.
4. Basic computer knowledge.
5. Valid Driver's license.
Experience:
One year experience with IDD population (preferred)
