Job Description:
Mountain CAP of WV, Inc., a CDC is seeking a dedicated and compassionate Case Manager to join our team. This position is full-time working with the Supportive Services for Veteran Families (SSVF) Program. The position will be based in the Buckhannon office but serve a six-county region in WV.

Duties will include providing strength-based case management services offering an atmosphere where Veterans in crisis can receive individualized assistance with housing, goal setting, and temporary financial assistance.

In addition, this position will be responsible for direct services to homeless veterans and their families, continued contact with landlords and community partners, maintaining current database and case file entries, and providing assistance in preparation of monthly reports.

This position requires a professional demeanor, excellent oral and written communication skills, strong organizational ability, and ability to maintain confidentiality. A bachelor’s degree or higher in social work or related field is required. Eligibility for a social work license in WV preferred. Requires clear CIB and APS/CPS background checks. A minimum of two years of previous case management or homeless service experience is preferred. Reliable transportation is required.
