JOB SUMMARY: This position involves the maintenance of an impaired individual in his/her own home. The primary function is to prevent the institutionalization of the client through the provision of long term maintenance of the client household and performing those personal care tasks that they are incapable of providing for themselves.
JOB RESPONSIBILITIES:
- Provide personal care services to impaired individuals to allow maintenance in their own homes and to prevent institutionalization.
- Follow plan of care developed for each client.
- Assist in maintenance of cleanliness and neatness in home environments including housekeeping chores.
- Assist in checking temperatures, pulse, respiration, blood pressure and weights and recording and reporting as required.
- Supervise the safety and well- being at all times.
- Demonstrate concern for all participants’ welfare by rendering immediate assistance as needed.
- Show sensitivity to individual needs, be aware of each individual’s abilities, and help individuals maintain self-worth and dignity.
- Consistently follow Standard Precautions when assisting clients.
- Consistently follow Body Mechanics training protocol in performing job tasks.
- Maintain daily logs and progress notes as prescribed by RN Supervisor and DMAS.
- Report all changes in client condition and personal schedule to RN Supervisor in a timely and accurate manner.
- Maintain electronic verification records and notify office promptly with any issue.
- Returns any outstanding personal aide records weekly.
- Report any suspected abuse, neglect, or exploitation to supervisor.
- Maintain up to date knowledge of given area of responsibility.
- Regularly attends meetings, as provided, by the Agency to ensure current knowledge of areas of responsibility.
- Research and/or recommend new process, procedures and applications which can better serve Agency or client needs.
- Displays an interest and enthusiasm in on-going Agency sponsored development activities; supports company efforts with consistent participation and attendance.
- Participates in Agency fundraising, as required by supervisor.
- Perform on the job to the best of ability at all times.
- Exhibit professional presence, consistent with Agency standards and values at all times.
- Interact with Agency personnel to facilitate positive, productive working relationships.
- Provide positive assistance and support at all times when dealing with staff and clients.
- Maintain regular and punctual job attendance, avoiding absences and schedule changes, when possible.
- Exhibit complete confidentiality at all times in all matters in regard to clients, their families, and other staff by observing all relevant Federal and state laws, rules, regulations and all ethical rules of conduct.
- Regularly checks email and responds appropriately.
- Adheres to deadlines on returning departmental forms to the office.
- Precept new hires.
ESSENTIAL FUNCTIONS
- Must have good interpersonal skills and relate well to older persons.
- Must be able to work independently with minimal support and supervision.
- Ability to adapt to daily schedule changes

QUALIFICATIONS:
- Must possess valid driver’s license and have ability to drive.
- Ability to read, write and maintain service records. Must be able to understand written orders.
- Physical ability to perform assigned duties.
- Satisfactory work record, as evidenced by, two satisfactory previous employment references.
- Completion of approved curriculum as a personal care aide, nurse’s aide, or certified nursing assistant.
- Annual completion of 12 hours of continuing education.
- CPR/First Aid training
- Medically cleared for communicable diseases with up-to-date immunizations before participant contact.
- Must pass pre-employment drug screening.
WORK SCHEDULE:
- Work schedule to include weekend work every other weekend
