Director of Patient Services – Hospice Home Care

Key Responsibilities
- Provide strong leadership, support, and supervision to nurses, aides, and medical records staff to ensure high-quality patient care and regulatory compliance.
- Oversee and monitor the quality of care, ensuring individualized, compassionate, and effective care plans for each patient.
- Ensure compliance with federal, state, and local regulations, as well as accreditation and hospice care standards.
- Collaborate with the interdisciplinary team to develop and coordinate comprehensive patient care plans.
- Build and maintain positive relationships with patients, families, and referral sources to promote exceptional hospice services.
Schedule
- Monday – Friday
- Day Shift: 8:00 AM – 5:00 PM
- Rotating Administrative Call
Qualifications
- Registered Nurse (RN) with an active license is required.
- Minimum of 3–5 years of leadership experience in healthcare, preferably hospice or home care .
- Strong ability to collaborate with interdisciplinary teams and lead with compassion and efficiency.
