We are looking for a dynamic and passionate Nurse Case Manager to join the Mountainside Team!
What You'll Do
The Nurse Case Manager will work within an interdisciplinary team and collaborate with physicians, family members, nurses, and other hospital staff throughout the patient's continuum of care. The Case Manager will provide compassionate quality care while assisting patients and families from pre-admission through discharge planning.
The successful candidate will be able to :
- Validate accurate status order to match appropriate level of care
- Proactively involved in discharge planning and ongoing assessments
- Secure appropriate certification from payers for services provided
- Prompt, accurate, and detailed reporting / documentation
- Review physician orders and patient progression on a daily basis and intervenes with care coordination as needed
- Plan for care need with active involvement of patient, significant others, hospital staff involved in treatment process;
- Application of process improvement methodologies in evaluating outcomes of care
- Coordination of communication with physicians, nursing, and staff of ancillary departments
- Evaluation of care provided against the Length of Stay
- Collaboration with Admitting to ensure accuracy of patient demographic and insurance information
- Communication with patients and their families around medical plan of care and discharge plan
- Assess and plan for discharge needs and arrangements, including leading multi-disciplinary care conferences in conjunction with Nursing
- Perform any and all duties assigned
