What are the responsibilities and job description for the Case Manager SUD and Mental Health position at Moving Mountains Recovery?
A Case Manager is a member of the clinical team, who is directly involved in providing quality care for clients. The Case Manager ensures client’s care by assessing treatment needs, developing, monitoring and evaluating the progress due to the initial assessment. The Case Manager will work with each client on services that will enhance the client’s well-being and community support. This individual must have a high degree of sensitivity to the agency’s target population and maintain ethical standards. This position requires initiative and independence in seeking out services for each client. The individual must be proficient in computer-based skills including Microsoft office, word, excel and power point as well as electronic medical record systems.
Required Education: HS Diploma
1 - 2 years' experience in Case Management
1-2 years mental health experience Job Responsibilities:
- Provide case management services in accordance with the individualized treatment plan;
- Assist in identifying needs that must be addressed while in treatment including housing, healthcare, employment, financial assistance, clothing, childcare, parenting, etc. and seek out necessary resources to address identified needs;
- Actively participate in the development and assessment of individual case management plans for clients in Partial Care Program;
- Maintains and utilizes knowledge of community resources and accessibility;
- Schedules weekly appointments with consideration of client needs and preferences;
- Coordinate with clinical team on individual client needs to educate, engage, and coordinate client care with their identified supports and community resources to ensure all services are implemented prior to completion of treatment;
- Complete all NJSAMS reports for all Partial Care Clients, including admissions, updates, transfers, and discharges;
- Serve as a backup for Behavioral Health Technicians, conducting intakes for new admissions for Partial Care;
- Collaborate and communicate with the client, family, significant others, identified support systems, community providers, and referral sources;
- Attends regularly scheduled consultations to ensure appropriate coordination of services.
- Refers clients to other professionals or services as appropriate to ensure optimal service, monitors and follows-up on status of referrals and existing services;
- Ensures that documentation is complete, accurate, timely and meets regulatory and agency standards;
- Maintains appropriate boundaries and displays respect and courtesy to all clients;
- Demonstrates patience, caring, compassion, and sensitivity toward clients;
- Attends meetings and supervisions as required and necessary for the position;
- Function as a Group Facilitator as needed
- Other duties as assigned.
