Population Health Director Woburn, MAThis position provides strategic planning, development, implementation, and leadership oversight for Marquis Health System's Care Management facility teams.The Population Health Director is considered a subject matter expert for the integration of system-wide Population Health initiatives.Assures the implementation of consistent care management practices including role functions, competencies, and performance management.Provides oversight, support, and coordination of care within the Skilled Nursing Care Management departments, Utilization Review functions and teams, and the facility Case Management department.Works closely with the Care Management teams across the health system within long-term care, home care, rehabilitation, and behavioral health. Works in partnership with other Care Management leaders in Complex and Chronic Care, Population Health Analytics, and Patient Throughput.Collaborates with corporate and health system team members to ensure that services are delivered within a culture that strives for excellence, respect, and continuous improvement, reflecting Marquis Health System's Mission, Vision, and Core Values.Roles & ResponsibilitiesDevelops structures and processes that contribute to continuity of care across the organization.Works collaboratively with internal and external resources and agencies, physicians, and all members of the team in meeting customer needs and organizational goals.Consistently creates and implements programs and processes that support seamless, patient-centric, best practice continuity of care in between multiple care settings.Programs are clinically integrated, incorporate the Voice of the Internal and External customers, ensure metrics that are Critical to Quality are part of the final product, and are sustainable.Successful demonstration of program / project management and data analysis skills in population health management. Programs and processes are consistent with change management and process improvement principles.
