MUST HAVE Experience in one of the following :
Working with pediatric population in Behavioral health setting 1-2 yearsDay to day responsibilities of this the role and a description of the project :
Staff will be responsible for managing a caseload of 75-85 members on the healthplan ages 3-21. They will conduct regular outreach (default monthly) and will perform assessments yearly and as needed to determine needs, care gaps, current services, satisfaction of services, and provide education to members and families of members on the healthplan.They will then coordinate any needed or existing care and monitor care for satisfaction and member for progress throughout the year. Yearly assessments will be conducted each year and as needed per change of condition and will be in person at member’s home or in an agreed upon public location that ensures HIPAA compliance.Staff will complete monthly calls and visits (telehealth or in person per member choice) to all members on caseload unless a different frequency of outreach is established and documented. Staff will work to remedy and circumvent member issues with providers when they arise. Staff will document all interactions in a timely manner as directed by internal work processes. Staff will staff appropriate cases with leadership when guidance is needed.Staff will provide follow up following member discharge from hospital to assess for needs, provide education, and coordinate services. Staff will work with local waivers, clinics, providers, and community entities to connect member with services covered and not covered by healthplan. Staff will maintain license and complete all activities required for this. Staff will communicate with leadership regarding any questions or need for assistance with any assigned duties.Describe the performance expectations / metrics for this individual and their team :
Make timely outreaches to members and complete assessments as scheduled and on time. Submit quality and timely documentation following any member interaction, outreach, assessment. Submit admin items (mileage, expense report, time sheet, etc) on time.What previous job titles or background work will in this role?

Case mgmt. experience 3 years; Healthcare experienceAbility to build rapport with pediatric members and familiesKnowledge of and experience with local resources under and not under healthplan for membersEducation Requirement :
