What are the responsibilities and job description for the Care Coordinator position at Mt. Hood Adult Day Center?
Mt. Hood Adult Day Center is looking for a dynamic individual to join our team of Care Coordinators/Case Managers. As a Care Coordinator with Mt. Hood Adult Day Center, you will be working with adults as an integral part of their care team to provide a place outside the home for older adults and adults with all types of disabilities to socialize with their peers.
The duties of a Care Coordinator include:
· Ensures all care folders are always up to date and meet or exceed state standards.
· Corresponds and builds relationships with care homes, family members, case managers and team members on participant’s care needs.
· Drive a program culture which encourages inclusivity, kindness, and respect.
· Keeps all living documents up to date. Including but not limited to:
· Participant care binders
· Employee emergency contact information
· Participant emergency care needs
· Fire drill binder
· Employee/Intern training documentations
· Participant Roster
· All participant protocol binder
· Responsible for keeping team members updated on participant needs and changing needs no later than the beginning of the following workday if not emergency, any participant needs that require immediate attention will be communicated verbally and in team binder.
· Maintain participant daily roster and ensure updated emergency information is current on the arrivals clip board ensure updated.
· Collaborate with team to ensure monthly fire drills are preformed and documented.
· Assumes Directors and/or Activities Coordinators position in the event of their absence.
· Assists with all care needs on the floor when needed to ensure the quality of the program.
· Ensure new tour files created as needed.
· Create new ISP materials and send to ISP team members 1 week prior to ISP meeting if possible.
· Attends meetings outside the program building including but not limited to:
· Entry meetings
· Behavioral meetings
· Medical meetings
· ISP meetings
· Support team meetings
· Intern meeting
· Home assessments
· Ensures I.R.s are written and sent to required team members within state guidelines of 24 hours.
· Meet with participant’s team members when necessary for ISP’s and behavioral challenges.
· Reports directly to Program Director.
· Responsible for writing Brokerage case notes, to be completed by the 5th of each month and then faxing to personal assistants by 10th of each month.
· Participate in monthly community relation meetings and trainings as needed.
· Creates new care folders for new participants and fills out all necessary paperwork before the participants first day of program.
· Creates monthly ADLs and goal sheets for all participants adding them to appropriate books on the last day of each month, adding completed ADLs to care folders by the 1st week of each month.
· Report safety concerns including I.R.s to Director immediately. Director to be included on IR report to case manager.
· Responsible for general health of program, ensuring program is safe - including but not limited to employee retention, participant retention and creating positive communication pathways.
· De-escalation of participants when safety is a concern.
· Assist in training new employees, ensuring all required training are met.
· Responsible for monthly employee schedule with correct ratios to be approved by program Director and discussed with team leads prior to submission.
· Review and discuss strategies to increase quality of care during weekly team meetings.
· Responds to all voice messages and emails within 1 business day.
· Collaborate with other care coordinators and case managers to create monthly outing calendars when applicable.
· Collaborate with floor lead to ensure healthy and fiscally responsible relationship with Meals of Wheels.
· Tour prospective new participants and create required documents for follow up.
· Collaborates with director for employee appreciation.
· Responsible for conducting in home assessment.
BSW in Social Work or 6 years of relevant work experience is required.
Job Type: Full-time
Pay: $24.00 - $27.00 per hour
