Department of County Human Services Aging, Disability & Veterans Services Division's mission is to promote independence, dignity, and choice in the lives of older adults, persons with disabilities, and veterans. As a Case Manager 1, you are determining initial and ongoing financial eligibility for Supplemental Nutrition Assistance Program, Medicaid programs, Temporary Assistance for Needy Families, and Employment-Related Day Care. You are a customer service expert, delivering benefit eligibility information with a trauma-informed lens. As a Case Manager 1 you will be using a wide variety of computer software programs along with implementing process improvement changes. Primary responsibilities include: Understand and apply Federal and State guidelines for Supplemental Nutrition Assistance Program (SNAP), Temporary Assistance for Needy Families (TANF), Employment-Related Day Care (ERDC), and Medicaid programs, including Oregon Health Plan, Medicare Savings Plans (QMB/SMB/SMF), Title XIX, and Employed Persons with Disabilities Conduct in-person, telephone, and field interviews with clients/applicants, and as necessary, with the applicant/recipient's relatives, guardians, and/or representatives to determine initial and ongoing eligibility for programs Accurate, appropriate, and timely data entry into various state computer systems to generate correct benefits Communicate effectively to maintain a shared caseload setting Record information, complete tasks, and respond to client inquiries Assist clients in the selection and use of health plans Make referrals as appropriate to internal and external community partners and organizations Successful case managers have: Respect and appreciation for ethnic and cultural diversity Knowledge of cultures, customs and social experiences of communities experiencing marginalization and barriers to opportunity Ability to multitask, prioritize, and stay current on clerical tasks Strong interpersonal, communication, and organizational skills Ability to make decisions based on specific rules and regulations Ability to complete work using standard software tools To Qualify We will consider any combination of relevant work experience, volunteering, education, and transferable skills as qualifying unless an item or section is labeled required.
