If so, Multnomah County invites you to apply!Aging, Disability and Veteran Services is excited to announce we are currently seeking an experienced individual to provide services as a Case Manager Senior. We currently have the following position available : Case Manager Senior (Nursing Facility DIVERSION Intake Case Manager) located at Tabor Square (4610 SE Belmont St. Portland, OR 97215) with Transition Diversion.The eligible list created from this recruitment will be used to fill Regular and Limited Duration positions. Limited duration positions can last up to 2 yrs, but may end at any time within the 2 yr period. The Intake Case Manager determines accurate initial and eligibility for Medicaid waivered services, Nursing Facility services, State Plan Personal Care services, Medicaid (medical) programs, Supplemental Nutrition Assistance Program (SNAP) and other state and federal programs. The Intake CM ensures that screening for Medicaid includes screening for all Medicaid programs and provides choice counseling. If applicant is not eligible for Medicaid programs, referrals are made to District Center or other appropriate community-based resources, as available. If and when possible this position will also move people from Nursing Facility care to the community. Their mandate is to quickly establish service eligibility or deny the service request. If eligible, CM will counsel clients of their CBC choices and then coordinate the transition out of the NF to CBC setting or make a referral for a Transition. If an applicant is not eligible for Medicaid programs, referrals are made to District Center or other appropriate community-based resources, as available.   • Equivalent to an Associate's degree from an accredited college or university with major course work in social science, behavioral science, or related field; -AND-Four (4) years of case management experience with a minimum of one year of experience at the full performance / journey case management level.Must pass a criminal background check
