The Opportunity

Multnomah County Health Department's Behavioral Health Division is seeking social workers and mental health professionals who represent the communities we serve and are dedicated to removing barriers to youth and families who experience intersectionality to join our team as the Clinical Services Specialist - Wraparound Care Coordinator - KSA Latine/Bi-lingual Spanish. The Wraparound Care Coordinator position is part of the Multnomah County Health Department's Behavioral Health Division. This position is responsible for leading a multi-disciplinary team to create individual plans to address the needs of children and families based on assessment and clinical evaluation of each client. The position also coordinates monthly team meetings and conducts investigations to help youth and families enrolled in Health Share of Oregon and experiencing complex challenges.

This is a position that requires advanced knowledge and experience with the Latine/a/o culture and community, specifically in the provision of mental health services, and the ability to speak, write, and read fluently in both the Spanish and English languages.

Clinical Services Specialist - $1,500 Hiring Incentive

As a newly hired Clinical Services Specialist, you will receive a one-time New Hire Recruitment and Retention Incentive payment of $1,500 on your first paycheck. The incentive will be prorated based on the position's FTE status

Teleworking/Hybrid: At this time, this position is designated to "routine telework/hybrid," meaning you will be working remotely (from home) most of the time and you will be required to attend meetings in the community. As with all telework positions, employees may be required to come into the office on occasion. The designation of Routine Telework may be subject to change at a future time. Currently, all employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations.

Note: The eligible list created from this recruitment may be used to fill regular, full or part time, temporary, and limited duration assignments.

As the Clinical Services Specialist - Wraparound Care Coordinator - KSA Latine/Bi-lingual Spanish duties include:

Wraparound Care Coordination

  • Facilitate the fidelity Wraparound process, including assessment and clinical evaluation of each client.
  • Investigate and apply clinical evaluation and skills to complex cases.
  • Lead the Wraparound process in collaboration with youth, family, and team members.
  • Coordinate access to necessary services. This includes, but is not limited to: completing referrals, educating providers on how to complete referrals, following up on the status of referrals and informing teams about this, and supporting teams in determining which services to pursue.
  • Lead Wraparound team meetings, complete assessment tools, and document findings.
  • Attend trainings and collaborate with child serving system partners.
  • Respond to new referrals within 24 hours and hold the first team meeting within 30 days.
  • Consult with the Wraparound leadership team and psychiatrist when needed.
  • Provide verbal and written services in Spanish and English.

Documentation

  • Complete CANS initial investigation and assessments within 30 days of enrollment to determine the child's needs, set a plan, and make decisions.
  • Lead CANS system collaboratively within a team approach and maintain partnerships with contracted agencies and providers.
  • Facilitate the linkage between information gathering and the design of individualized service plans.
  • Update CANS scoring sheet every 90 days and update the Plan of Care based on ongoing investigations and clinical evaluations.
  • Enter detailed service notes into Evolv within 24 hours of service delivery.
  • Update Plan of Care monthly and submit to supervisor or coach and team members within 5 business days of the team meeting.
  • Lead evaluation of care plan updates within the team and lead the development of needed changes or improvements to the plan.
  • Complete CIM and Evolv data entry to accurately reflect treatment requests for authorizations.
  • Complete Strengths & Needs Summaries, Crisis & Safety Plans, Plans of Care, and Transition Plans within required timelines.

  • Complete required County and State training and adhere to policies.
  • Participate in supervision and training.
  • Maintain values of Wraparound Program and track case assignments.
  • Balance workloads, consult on cases, and monitor service delivery.
  • Conduct work professionally and respectfully.
  • Complete work ethically and submit timely documentation.
  • Attend program meetings and trainings.
  • Meet with Wraparound Supervisor

Required Minimum Qualifications/ Transferable Skills*:

Education/Experience

  • Equivalent to an Associates degree in social services, psychology, sociology, counseling or related field and four (4) years professional work experience as a counselor, advocate, or social worker ; OR,
  • Equivalent to a Bachelor's degree in counseling, social services, psychology, sociology, social work or a related field and two (2) years of professional work experience as a counselor, Mental Health treatment professional, social worker, or therapist; AND

Certification

  • Must meet the standards of Qualified Mental Health Associate - (QMHA) with the Mental Health & Addiction Certification Board of Oregon (MHACBO) - more information can be found at MHACBO website. Current QMHA (Required)

Other

  • Spanish and English (language testing is required upon hire)
  • Valid Driver's License (Required)
  • Advanced knowledge of and experience working with the Latine/a/o and Spanish-speaking community. This would be demonstrated by knowledge of the traditions, values, and customs of the community being served as well as the ability to identify specific examples of experience working directly with individuals and families who identify with the Latine/a/o and Spanish-speaking community.
  • Understands cultural/racial/ethnic factors, including systemic disadvantage; acculturation, intergenerational immigrant family challenges, effects of trauma during immigration, war, separation from family/primary supports, detention, generational trauma, and any factors related to non-dominant culture affecting mental health/substance abuse and treatment.
  • This position requires a Background Investigation, which may include being fingerprinted.

Preferred Qualifications/ Transferable Skills*: You do not need to have the following preferred qualifications/ transferable skills to qualify. However, keep in mind we may consider some or all of the following when identifying the most qualified candidates.   • Working knowledge of Google Suite, EvolvNX and Epic databases preferred.
  • Experience working within the youth and adult systems of care, including crisis and acute care settings, shelter and housing, and mental health services. .
  • Experience working with individuals who have Oregon Health Plan, Health Share/CareOregon, and Trillium Centene Health Plan.
