Job Title : Community Health Worker
Position Location : Remote – Candidates must be located around Lehigh, PA
Work schedule : Monday-Friday with weekend work once a quarter
Field Travel needed over 50% of the time (Travel will be reimbursable)
Job Description :
The Community Health Worker (CHW) will be responsible for helping members to navigate and access medical care, community services / resources and develop healthy behaviors. The CHW will act as a caring neighbor to help members address the social, medical and community problems that lead to poor health (SDoH / HRSN). As a priority, interactions will promote, maintain, and improve the health of members and their families. This includes to provide social support and advocate for individuals and community health needs.
- Establish trusting relationship with members.
- Make follow up calls and community visits to ensure member is connected to community services.
- Meet member in medical setting to ensure member has comprehensive and coordinated care.
- Assist member with completion of applications for medical or social benefits.
- Document all member encounters in documentation platform.
- Maintain excellent working relationships with members, families, providers and CBOs.
- Knowledge of community resources
