Job Summary
The Director of Case Management is a pivotal leadership role responsible for overseeing the case management department within a healthcare setting. Responsibilities

  • Develop and implement case management policies and procedures to ensure high-quality patient care.
  • Supervise and mentor case management staff, providing guidance on best practices in patient assessment and care coordination.
  • Collaborate with interdisciplinary teams to create comprehensive care plans tailored to individual patient needs.
  • Monitor compliance with Medicare regulations and hospital policies related to case management.
  • Conduct regular reviews of case management processes, identifying areas for improvement and implementing necessary changes.
  • Serve as a liaison between patients, families, and healthcare providers to facilitate effective communication and support.
  • Provide expertise in addiction counseling and physiology knowledge to enhance patient care strategies.
  • Analyze data related to patient outcomes and resource utilization to inform decision-making.

Skills

Job Types: Full-time, Contract

Pay: $135,702.00 - $165,138.00 per year
