What does success look like as a Behavioral Health Nurse Practitioner?
- Comprehensive Psychiatric Assessments: Conduct in-person or telehealth assessments and collaborate with the IDT to develop personalized care plans
- Collaboration with Mental Health Counselors: Partner with the Mental Health Counselor to guide IDT meetings, bridging knowledge gaps on behavioral health issues, and empowering the team with tools for effective interventions
- Behavioral Health Education: Educate IDT members on mental health disorders, substance abuse, dementia-related behaviors, and goal setting for meaningful behavioral interventions
- Integrating Behavioral Health Care: Work closely with clinicians to integrate behavioral health into participants' care plans seamlessly
- Building Community Partnerships: Establish and maintain strong communication with community behavioral health specialists, ensuring collaborative care
- Coordination with Geriatric Psychiatrists: Manage schedules and facilitate the exchange of critical information to make the most of geriatric psychiatric consultations
- Inpatient Follow-Up: Support participants admitted to psychiatric facilities to ensure issues are addressed, and follow-up care is well-planned
- Behavioral Health Diagnosis: Evaluate participants with complex behavioral health issues to develop accurate diagnoses and tailored care plans
- Team Training & Support: Provide behavioral health education to team members, equipping them with knowledge to enhance participant care
- Contribute to Organizational Growth: Embrace additional opportunities and responsibilities as needed, actively participating in the continued growth and success of the organization
- Passion for serving high-risk seniors and frail older adults
- 3 years of experience providing behavioral healthcare to elderly or complex populations
- Experience working with Interdisciplinary Teams in a healthcare setting
- Active Psychiatric Mental Health Nurse Practitioner State License without restrictions
- State CDS Certification and DEA License
- NPI License required
- PACE experience preferred but not required
- CPR and First Aid certification (or ability to obtain within 60 days)
- Valid CA driver’s license, personal transportation, good driving record, and auto insurance (as required by state law)
Salary : $130,000 - $180,000
