About This Role

As our Enrollment RN Manager, you’ll help make that vision a reality by guiding prospective participants through one of the most important transitions in their care journey—enrollment into our PACE program.
In this role, you’ll lead a dedicated team of Enrollment RNs, ensuring every potential participant receives thoughtful, timely, and clinically sound assessments. You’ll collaborate closely with Outreach, Center Leadership, and other internal partners to align enrollment efforts with program capacity and community needs, all while maintaining the highest standards of clinical quality and compliance.

\n- Lead with Heart and Purpose: Supervise, mentor, and support the Enrollment RN team to deliver high-quality, person-centered care during the enrollment process. Create a culture of accountability, growth, and continuous improvement.
- Collaborate Across Teams: Partner with Outreach, Center Leadership, and internal departments to align enrollment operations with organizational goals, capacity planning, and community needs.
- Ensure Clinical Excellence: Oversee timely and accurate Nursing Facility Level of Care (NFLOC) assessments. Guide the submission and management of clinical documentation to the Department of Health Care Services (DHCS), ensuring compliance and timeliness.
- Champion Accuracy: Maintain exceptional standards for documentation in Electronic Health Record (EHR) and Customer Relationship Management (CRM) systems, supporting audit readiness and data integrity.
- Be a Knowledge Hub: Serve as a subject matter expert in PACE eligibility, enrollment regulations, and state/federal requirements. Develop and lead training to keep the team confident, consistent, and compliant.
- Drive Connection and Coordination: Collaborate closely with teams across Outreach, Finance, Compliance, and Center Operations to ensure smooth, efficient, and compliant enrollment workflows.
- Use Data to Guide Action: Monitor and interpret key metrics—such as RN productivity and documentation timelines—to inform strategy and identify opportunities for improvement.
- Support and Present with Confidence: Lead clinical eligibility review meetings and present RN recommendations with clarity and regulatory insight.
- Step In When Needed: Provide direct RN-level support, including home assessments and NFLOC evaluations, to ensure continuity and meet participant needs.
- Stay Ahead of Change: Monitor and implement updates in eligibility criteria and regulations, adjusting internal processes and training as needed.
- Embrace Innovation: Lead efforts to optimize EHR and CRM systems, reducing administrative tasks and enhancing data visibility.
- Develop Clinical Excellence: Coach team members on documentation, safety assessments, and clinical decision-making to ensure every assessment meets our high standards.
- Identify and Act on Risk: Proactively surface and address clinical, operational, or regulatory risks to protect the participant experience and ensure timely enrollment.
- Contribute to the Bigger Picture: Step into special projects or initiatives as needed to support our mission of delivering compassionate, all-inclusive care for older adults.
- Heart for the Mission: A deep commitment to supporting frail older adults and helping them remain safe, healthy, and independent in their homes.
- Nursing Education: Associate’s Degree in Nursing required; a BSN is strongly preferred.
- Licensed in California: Active, unrestricted RN license in the state of California.
- Experienced Caregiver: At least 5 years of nursing experience, ideally with older adult populations in home or community-based settings.
- Team Leader: 2 years of supervisory or management experience—whether guiding clinical teams, mentoring RNs, or overseeing performance.
- PACE & Value-Based Care Knowledge: Familiarity with PACE, Medi-Cal, or similar value-based care programs is a plus.
- Strong Communicator: You bring excellent interpersonal, leadership, and communication skills to foster trust and clarity across teams.
- Tech-Savvy and Organized: Proficient in Electronic Health Records (EHR) and Customer Relationship Management (CRM) systems—comfortable using data to manage workflows and drive decisions.
- Driver's License and Transportation: A valid California driver’s license and access to reliable transportation.
- Language Skills: Bilingual abilities (such as Spanish, Cantonese, Mandarin, or Vietnamese) are highly preferred and deeply valued in our diverse communities.
- CPR Certified: CPR certification is required (or can be obtained within 60 days of hire).
- Schedule: This is a full-time position, Monday through Friday, offering consistent daytime hours that support a healthy work-life balance.
- Work Environment: The role follows a hybrid model, blending some remote-based tasks with field-based responsibilities. You can expect to travel up to 100% within your assigned service area to conduct assessments, lead teams, and stay close to the heart of participant care.
