About myPlace Health

myPlace Health was founded in 2021 by mission-aligned healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. We specialize in providing value-based, comprehensive care and coverage for older adults with significant needs so they can thrive in the homes they love and in the communities they cherish.

Our mission is simple: to enable older adults to live the independent lives they deserve. We pursue this mission through our myPlace PACE (Program of All-Inclusive Care for the Elderly) model, which provides seamless primary care, integrated health plan coverage, personalized social engagement, and customized services delivered in the participant’s preferred place.

myPlace Health is building a mission-driven team that shares our passion for redefining the way older adults experience care as they “age in place” in the community. This is a unique opportunity to take on one of our country’s most challenging healthcare problems and join a fast-growing, dynamic team as we prepare to scale our mission to serve more markets.

About This Role

At myPlace Health, we believe everyone deserves the chance to age with dignity, independence, and connection. As our Enrollment RN Manager, you’ll help make that vision a reality by guiding prospective participants through one of the most important transitions in their care journey—enrollment into our PACE program.

In this role, you’ll lead a dedicated team of Enrollment RNs, ensuring every potential participant receives thoughtful, timely, and clinically sound assessments. You’ll collaborate closely with Outreach, Center Leadership, and other internal partners to align enrollment efforts with program capacity and community needs, all while maintaining the highest standards of clinical quality and compliance.

This is a meaningful leadership opportunity for someone who is not only a skilled and compassionate RN, but also a team builder, systems thinker, and advocate for older adults. You’ll have the chance to shape and continuously improve the enrollment experience—making it smoother, safer, and more person-centered every step of the way.

If you're passionate about helping frail older adults stay safe at home, energized by cross-functional teamwork, and ready to lead with purpose and care, we’d love for you to join us.
