Position Overview:
As an Advocacy Intake and Exit Coordinator, you will be responsible for facilitating the intake process to ensure a smooth transition for clients entering into MYSI’s residential programs. This coordinator position is also responsible for coordinating the process for youth exiting the program. The Advocacy Intake Coordinator responds to all intake inquiries (via email stream & telephonic) and completes initial screening for new clients. The intake coordinator ensures the complete referral packet is received and schedules initial intake appointments with the appropriate program leadership, while maintaining timely communication via the email stream regarding disposition, and requesting placement documentation if applicable.. The coordinator will also work with the case management team to prepare youth to exit the program upon emancipation or any type of exit. The coordinator will assist with the Count Down to 21 process and provide support for these crucial transitions.
Job Duties:
- Maintain superior knowledge of MYSI programs, including matching checklists and alternative community resources. .
- Respond to/receipt of all referrals within one business day.
- Ensure complete referral packet is received within 2 business days
- Schedule and coordinate the pre-placement visit within 3 business days of receiving the complete referral packet. Ideally, this visit should be in-person. Receive feedback information on the outcome of the visit.
- Communicate timely via the email regarding interview dates, dispositions, etc. within the DCFS stream.
- Maintain and submit weekly intake report, including managing waitlist data.
- Manage influx of new clients and program size to adhere to DCFS regulations and rules.
- Proficient use of MYSI electronic systems. (Training available)
- Inform necessary departments of the arrival of new clients and coordinate the transition and admission process as outlined in the residential handbook.
- Help lead the exit process partnering with case management and residential staff to ensure a timely and smooth exit. This can include coordinating dates, arranging transportation, helping coordinate the move of the youth items and ensuring a plan for the pick up/drop off of the youth on the day of the move.
Requirements:
- Must be able to administer CPR, First Aid and CPI.
- Travel as required
- Must meet DCFS background and transportation requirements.
- Must comply with Childcare Act CH 23 part 2215.1 Section 5.1a 6.
What you will bring to MYSI Corporation:
- High School Diploma or equivalency.
- Three Years of professional experience in a social service setting, including at least one year working with youth or individuals with development disabilities and some experience working in a residential setting.
- At least some experience working with communities facing disparities.
- Comprehensive computer skills, including Microsoft Office 365.
- Strong technical and analytical skills, problem solving and conflict resolution skills.
- Valid Illinois driver's license
- Proof of valid automobile insurance
- Must be able to successfully pass background check.
