NAFI NY provides innovative community-based services to children and families in need across Westchester or Orange Counties.
The Parenting Love and Limits (PLL) program is seeking a compassionate, dedicated Case Manager to provide case management services to families in Westchester County or Orange County. The Parenting Love and Limits Case Manager / Bilingual Case Manager as a pivotal role in supporting families to achieve goals, address needs that lead to DSS involvement and support safety and well-being children and families. To engage and effectively work with families within their natural environment the Case Manager / Bilingual Case Manager provides services in the family's home and community. The case planner is an integral role responsible for supporting families, collaborating with community providers, connecting family to services, and ensuring safety and well-being.
Parenting Love and Limits program is an evidence-based practice that can reduce residential lengths of stay and can also be used to prevent residential placement for teens. Clients and their families participate in a six-week group and a minimum of eight to twelve family therapy sessions. The Case Manager / Bilingual Case Manager works in conjunction with NAFI PLL clinician to support families with addressing needs and goal attainment.
The skill required to be a dynamic Case Manager include :
- Complete intake screenings of new referrals in accordance to NAFI NY and PLL standards
- Complete pre and post assessments according to NAFI NY and PLL standards
- Facilitate PLL groups in conjunction with PLL clinician
- Provide respite support activities for youth on caseload
- Provide support and guidance to families to achieve service plan and treatment plan goals
- Provide linkages to community services to the family to ensure that they receive services needed
- Provide services to families at times convenient to them including evenings and weekends
- Conduct minimum required visits with families each month as per NAFI NY, PLL, DSS and OCFS requirements
- Complete PLL model adherence and fidelity measures
- Attend PLL scheduled consultations and incorporate feedback into work with families
- Collaborate with Department of Social Services on family's needs and services being provided
- Team approach to connect families with service providers to support families achieving goals
- Assist with crisis management, intervention and de-escalation when needed
- Document progress notes, reports, treatment / service plan that demonstrate the case management support provided to families including success and challenges of the family progress towards goal attainment
- Report both written and orally to stakeholders and courts on the progress and barriers the family is experiencing
