We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team as a Clinical Support Specialist in our assisted living program. This role offers a unique blend of direct patient care and leadership responsibilities, perfect for an LPN looking to advance their career.
Key Responsibilities:
- Implement care plans and administer medications and treatments for assigned residents
- Assumes responsibilities for the care of assigned residents, as needed, based on staffing fluctuations
- Prepares and administers medications, medical and nursing treatment, and nursing treatment to meet medical orders and the interdisciplinary plan of care as assigned.
- Leads development of overall resident’s service plan utilizing an interdisciplinary team approach
- Provides team member training related to each resident’s service plan
- Provide clinical oversight and leadership in resident care management
- Assume direct care responsibilities as needed, adapting to staffing fluctuations
- Assist with onboarding new nursing team members
- Ensure completion of clinical training competencies during orientation
- Evaluate ongoing clinical skills of nursing staff
Ideal Candidate:
- Current LPN license in good standing
- Strong clinical skills and attention to detail
- Excellent communication and leadership abilities
- Experience in assisted living or long-term care settings preferred
- Passion for geriatric care and continuous improvement

Requirements
- Associate Degree in Nursing required
- LPN licensed in the state employed
- Minimum 3-5 years of experience in a hospital, long term care facility or other related health care facility
- Must possess strong written and oral communication skills; able to maintain confidential information
- Must be proficient in Microsoft Office
