National Pike Health Center is a community-based Outpatient Mental Health Center with offices in Catonsville, Dundalk, and Westminster, MD. We provide mental health therapy and a psychiatric rehabilitation program for all ages and groups. We have a current opening for a Rehabilitation Specialist for minors in our Psychiatric Rehabilitation Program (PRP) at our Westminster location. We look forward to you joining our team!

If you have a certification for CPRP, you MUST have the certification for a CFRP(Child and Family Resiliency Practitioner) as well, to work with MINORS.
Overview of Job Duties
Developing and assuring that rehabilitation services meet the needs of the individuals served by the program; Providing oversight for the daily program of rehabilitation services by:
- Review and Process Completed Intakes and Assessments
- Completion and Maintenance of Individual Rehabilitation Plan
- Facilitation of Therapist Collaboration Meetings
- Process Discharges
- Counselor Supervisions
- Attend PRP Meetings.
- Completion of Weekly Report

Responsibilities
Required Duties:
Review and Process Completed Intakes and Assessments.
Ensure that the chart is COMAR compliant
Create a plan to obtain any/all missing documentation
Obtain Authorization
Work with PRP Director to assign all clients once authorization is obtained
Add new client to the database & ensure that all information is entered in Insync
Completion and Maintenance of Individual Rehabilitation Plan
(every 3mos/children; 6mos/adults).
Initial plan should be completed at the time of assessment.
Initial plan should be discussed during supervision with counselor.
90 & 180 day review plans should include advocate, parent/guardian and client. (phone is acceptable)
Date for 90 or 180-day review is listed on the database.
Document all unsuccessful attempts in Valant on a contact log.
Completed 90 or 180 day plans should be listed on the weekly report.
Facilitation of Therapist Collaboration Meetings. (every 6 months)Contact and document communication with therapist. (Completion of Continuation of Services Referral Form; can be delegated to counselor)
Meeting must include supported documentation with the rehabilitation specialist, referring clinician (phone acceptable), counselor, parent/guardian and client. (Completion of Therapist Collaboration Form)
Attendance at MDT (Multi-Disciplinary Team) meetings when appropriate
Reauthorizations should be completed between the 15th and the 21st of the month
Scheduling of re-authorizations should be reflected in weekly report & database updated
Document all unsuccessful attempts in Valant: use code 0000 or none at all
Completed re-authorizations should be listed on the weekly report.
All paperwork is due every Friday by 9:00 p.m. Failure to do so will result in a PIP.
Discharge ProcessDuring supervision attempt to contact the family/client/guardian
Follow up with referring therapist, include Director in all communication
Send out 10 day intent to discharge
Email notification of current discharge list to PRP Director and Clinical Director
Completion of discharge summary within 48 hrs. in Valant
Discharge the authorization in Value Options
Document completed discharges on Weekly Report.
Inactivate in Valant if not receiving other services at NPHC
Counselor SupervisionConduct & Document monthly supervision with assigned counselors on the Supervision Form. Supervision can be: Individual or Group
Report ALL counselors not in compliance with supervision
Write PIP for clients who are non-compliant
Ensure all notes follow the BIRP format and are consistent with goals in treatment plan; ensure that monthly summaries include updates on every goal from the IRP
Sign off on all supervisee notes in Valant prior to the next PRP pay period.
Attend ALL PRP Meetings.Weekly administrative meeting
Monthly departmental meeting
Monthly all staff meeting
Bi-weekly MDT meeting when appropriate
Completion of Weekly Report
Submitted every Friday by 9:00 p.m. to PRP Director. Failure to report will result in a PIP.
Review of census and weekly report with Administrative staff.
All paperwork is due every Friday by 9:00 p.m. Failure to do so will result in a PIP.
Based on the current needs the following schedule will be implemented:1st two weeks of the month: Complete ALL supervision sessions
15th-21st of the month: Complete ALL reauthorizations
Friday: Review all new intake charts to be filed
Requirements
Qualifying applicants must meet the following qualifications:
- Licensed mental health professional (LCSW-C, LCPC-S, LCSW, LCPC, LMSW, LGPC), Certification as Rehabilitation Counsellor (CRC) or CPRP-Certified Psychiatric Rehabilitation Practitioner; CFRP - Child and Family Resiliency Practitioner
- 2 years of relevant experience
