Patient Wellness Coordinator

Key Responsibilities:

1. Perform dressing changes and wound care, monitoring ulcers and incisions with accurate documentation and communication to physicians and families.
2. Assess weekly nutrition and hydration needs of residents with ulcers and incisions, evaluating and initiating additional nutritional intake as required.
3. Monitor lab reports for incidence of infection in assigned wounds, identifying causative factors and implementing solutions, and communicating with nursing staff and management.
4. Augment floor staffing as needed to ensure seamless delivery of care.

Qualifications:

• Active license as a Nurse in good standing with the state agency.
• Excellent technical, assessment, and documentation skills, leadership qualities, and effective communication skills.
