Job Title: Community Health Navigator - Patient Support Services
Duties:
- Support patients in accessing medical care by coordinating and attending appointments as needed.
- Promote medication adherence through patient education and addressing barriers.
- Provide coaching tailored to individual and population-specific needs.
- Act as a liaison between patients and healthcare teams to ensure care plan adherence.
- Manage a caseload of up to 20 clients, providing regular health education and follow-up.
- Conduct entitlement reassessments and handle logistics like appointment reminders and transportation.
- Accurately document patient interactions and input information into a web-based system.
- Perform other tasks as needed to meet program objectives.
Skills Required:
- Strong verbal and written communication skills; Spanish language skills are a plus.
- Detail-oriented with a focus on timely and accurate documentation.
Pay Range:
- Weekly: $1,250 - $1,500
