Job Title: Health Services Coordinator - Patient Liaison
Duties:
- Guide patients through their medical and support service plans, including arranging and attending appointments.
- Monitor and address issues related to medication adherence.
- Provide population-specific education and coaching tailored to program needs.
- Act as a liaison between patients and healthcare providers, ensuring plan compliance.
- Maintain a caseload of up to 20 clients, delivering consistent health education encounters.
- Coordinate logistical support for appointments, including transportation and childcare.
- Accurately document patient interactions and enter data into electronic systems.
- Perform related tasks to support departmental goals.
Skills Required:
- Excellent communication skills, with preference for Spanish speakers.
- High level of accuracy and timeliness in documentation.
Pay Range:
- Weekly: $1,300 - $1,550
