Healthcare Social Worker - Mental Health Services
Qualifications:
- L.M.S.W., L.C.S.W., or eligibility for licensure
- At least three years of post-master’s experience in a hospital, clinic, or behavioral health environment
Duties:
- Assess patients' psycho-social needs and provide intervention strategies
- Develop individualized care and discharge plans
- Assist families and patients with navigating community resources and post-hospitalization care
- Collaborate with healthcare teams to ensure holistic treatment plans
- Additional responsibilities based on department needs
Pay: $60 - $65/ hourly
