Position title Shelter/RRH Stabilization Case Manager Description
Will facilitate Scott Place Shelter case management by providing seamless transition of individuals experiencing homelessness into affordable, quality standard housing. The position will provide a housing-focused plan and implement stabilization services to assure a rapid transition to permanent housing.
Education / Experience
Associate or bachelor’s degree (preferred) from an accredited college or university in human services/social services field AND/OR minimum of 3 years documented work experience in case management working with homeless individuals, families, and or homeless service programs is preferred.
Physical Demands
While performing the duties of this job the employee occasionally is required to stand, walk, sit, use hands, drive an automobile, talk, and hear. The employee must be able to lift and/or move up to 25 pounds. Can walk up and down steps 4 or more times daily.
Job Requirements & Responsibilities
Be able to provide own transportation; carry adequate insurance coverage, Criminal Background Check; Protective Services Record Check; TB Risk Assessment or TB screening by Mantoux method. Must be able and willing to work any day &/or evening shifts; possible night shift periodically if or as required by program needs; may include some holidays. Some Day &/or overnight travel may be required for training purposes.
Excellent Computer knowledge, ability to quickly build rapport, strong written, verbal, and non-verbal communication skills, ability to demonstrate empathy plus navigate between being assertive and being supportive. The position requires the ability to work as a team member with other staff across all programs, maintain a good attitude and behavior with all staff and clients. Must present a professional appearance, be dependable and productive in the work environment. The programs require a high ability to learn and grasp instructions while meeting ever-changing conditions through problem solving capabilities. Maintain confidentiality of client matters and adhere to proper agency, state, and federal privacy guidelines.
License/Training/Certifications
Valid Driver's License. Must obtain CPR/FA cards and any other certifications that may be required for the position; to be obtained within the first 3-6 months of accepting position. *Some certifications are provided/paid for by employer. Social Work license preferred.
