What are the responsibilities and job description for the Direct Care Associate position at ND Health and Human Services?
South Country Human Service Zone supports the elderly and/or disabled community that wants to continue to be successful in their own home environment. As a Direct Care Associate, you will connect with citizens of the State of ND to complete daily living activities with the goal of increasing their ability to be independent.
Direct Care Associates will provide direct service to the elderly and/or disabled individuals in their home to enable them to continue to reside in their home/community. In this role, you will be responsible for providing environmental services and personal cares to eligible individuals, such as: laundry, shopping, housekeeping, meal preparation, transportation, bathing, hair care, etc.
You will need to have attention to detail in this role because you will be responsible for accurately documenting activities through electronic visit verification and/or paper logs; be able to manage your time and priorities effectively and pride yourself on excellent communication skills, empathy, and being a team player.
Applicants should have strong verbal and written communication skills, computer literacy, collaboration, and decision-making skills. You will have a passion for helping others and making a difference in our community.
This position requires daily travel to client homes throughout Emmons, McIntosh, Logan, Kidder Counties, as well as Dickey and LaMoure Counties when needed. Individual would participate in staffing and required trainings to maintain Qualified Service Provider status.
South Country Human Service Zone partners with the Department of Health and Human Services and McIntosh County to deliver state-driven child welfare safety and program protocols and provides employees with the excellent benefits and employment packages offered by Emmons County as an Emmons County employee.

Minimum Qualifications
Must have a high school diploma or GED and two (2) years of work experience providing direct care to special populations. Active CNA license preferred.
South Country HSZ will consider Direct Care Associate underfill, which requires a high school diploma or GED and one (1) year of work experience providing direct care to special populations. Active CNA license preferred.
The selected candidate must successfully complete a standard background and criminal record check. The selected candidate must have a valid driver's license with clean driving record, a reliable vehicle for transportation, and adequate auto insurance coverage.
