Your Role

Qualifications

• Education Requirements:   Graduate of an Accredited Medical School.  Completed an Accredited Post-Graduate Training Program in Family Medicine.  Board Certified or Eligible in Family Medicine.
• Required Licensure and/or Certification:  Current Illinois State License, Controlled Substance License & DEA.

Compensation 

Compensation is determined based on each candidate’s experience, skills, and education, within the established range for the role. Candidates who meet the minimum requirements will typically start at the entry level of the range, with additional qualifications reflected in the final compensation package.
