Case Management Director


Summary: Neighbor to Neighbor, Inc. (N2N) is a small nonprofit dedicated to providing social services, support, and friendship to refugees and immigrants who move to the South Bend community. We are seeking a compassionate and dedicated Case Management Director to join our team. As a Case Management Director, you will play a crucial role in assessing clients’ needs, developing care plans, and coordinating services to help them achieve their goals with the accompaniment of Neighbor to Neighbor volunteers. Job Description: We are looking for an experienced case manager to provide comprehensive case management services to our clients. Your primary responsibility will be to assess clients’ needs, develop individualized care plans, and coordinate services and resources to meet those needs. You will collaborate with clients, their volunteer team, N2N staff, and other professionals to ensure the delivery of quality care and support. The ideal candidate is empathetic and organized and has a strong commitment to newcomer care.


Responsibilities:

  • Conduct comprehensive assessments of clients’ needs, strengths, and goals.
  • Develop individualized care plans in collaboration with clients and their volunteer team.
  • Coordinate and monitor services and resources to meet clients’ needs.
  • Provide advocacy for and emotional support to clients and their families.
  • Collaborate with healthcare providers, social service agencies, and community resources.
  • Document client interactions, progress, and service delivery accurately and in a timely manner.
  • Evaluate the effectiveness of interventions and adjust care plans as needed.
  • Stay updated on industry best practices and maintain professional knowledge.
  • Assist in overseeing interns in social work programs.


Qualifications:

  • Bachelor’s degree in social work is required. MSW is preferred. 
  • Proven experience as a Case Manager in social services or a related field.
  • Strong knowledge of case management principles, practices, and regulations.
  • Excellent assessment and problem-solving skills.
  • Compassionate and empathetic approach to client care.
  • Effective cross-cultural communication and interpersonal skills.
  • Ability to work collaboratively and independently.
  • Proficiency in case management documentation.
