Position Summary: The Project for Assistance in the Transition from Homelessness (PATH) Outreach Coordinator serves as the program’s primary contact. The PATH Outreach Coordinator oversees the PATH Outreach team and SUD Specialist as they seek out, interview and assist persons who are homeless and have substance use or mental health disorders. The PATH Outreach Coordinator maintains a small caseload and will work in a variety of settings in five days a week including non-traditional hours looking for people in areas not meant for habilitation: parks, wooded areas, abandoned homes.
Supervisory Responsibilities: PATH Outreach team and SUD Specialist
Essential Job Functions:
· Oversees and supports employees of the PATH program
· Maintains schedules for shift coverage and ensure a program rules and staff responsibilities are adhered to
· Conducts outreach with homeless individuals identified in various community settings: streets, shelter, day resource centers, library, etc.
· Assesses and documents individuals for program eligibility. (i.e. homelessness, substance use disorders).
· Provides information and referrals to community resources, including mental health, AODA, medical, educational, financial services, based on assessment.
· Follows up to ensure clients are connected and utilize referral services.
· Advises clients regarding immediate needs, such as food and shelter.
· Identifies and responds in emergency situations (mental health, physical, weather-related, etc.).
· Assists individuals who are applying for Social Security Disability and other benefits.
· Transports and accompanies clients to appointments, using public transportation or personal automobile, as needed.
· Ensures case records are available for generating reports, audits, and grant writing.
· Enters all necessary client data into HMIS.
· Uses Case Management tool to track statistical and geographical information which is not entered into HMIS for monthly reporting.
· Creates and monitors incident reports while providing assistance to staff regarding proper incident reporting procedures
· Responsible for communicating incidents, concerns and/or events to Manager of S&ES
· Maintains client files for accuracy and compliance
· Compiles statistics and prepare reports as assigned
· Maintenance of inventory and ordering of supplies
· Attends trainings and community based meetings as required representing Neighborhood Alliance.
Additional Job Functions:
· Maintains HMIS (Homeless Management Information System)
· Provides oversight for Data Entry for billing, HMIS Database, and PDX report
· Checks and maintains current roster of participants and participant needs
· Verifies requests for rent payment and submits to accounting
· Coordinates with other Neighborhood Alliance programs to support clients
· Performs other duties as assigned
Qualifications:
· High School Diploma, GED Equivalent required, Bachelor’s degree preferred
· Certification or additional training in SUD preferred but will train right candidate
· Must demonstrate good writing and verbal skills
· Computer skills a must: Data Entry, Microsoft Excel and Word
· Experience working with low income individuals and families preferred
· Experience working in the mental health field highly desirable
· Ability to remain calm and think clearly in a crisis is essential
Physical Demands: In order to perform the essential functions of this position successfully the employee may be required to sit, stand, reach, lift, bend, kneel, stoop, climb, push and pull items weighing 50 pounds or more. This position requires manual dexterity, auditory and visual skills, and the ability to follow written or oral instructions and agency procedures. All functions must be able to be performed with or without reasonable accommodation.
Typical Working Conditions: This position operates in a homeless shelter/residential type environment as well as in the community of program participants. This position operates both indoors and outdoors on an ongoing basis. Exposure to the elements is common in this position. The employee may be exposed to heat/cold, steam, dust, cigarette smoke, pet dander, bacteria, noise, and communicable diseases. The employee may be exposed to loud noise and potentially volatile situations.

Pay: $35,568.00 - $40,000.00 per year
